PATIENT REFERRAL
Share the exceptional patient care you receive from Mountain Dental. Refer a patient to Mountain Dental and we’ll thank you both with a $20 gift card of your choice to either Target or BP (gas station).
Current Mountain Dental patients must complete a referral card and give to new patient to complete and bring in at the time of their first appointment. If a patient has not been seen at a Mountain Dental location for 2 years or more they will qualify as a new patient if they return.
